With the nurse case manager, manages patients based upon service lines from pre-hospital through post- discharge care.

Responsibilities

  • 1. Collaborate with the nurse case manager and other members of the health care team in the development of the discharge plan for the patient.
  • 2. Provide interventions regarding issues impacting access to continuum of care services.
  • 3. Provide advocacy and counseling regarding essential services.
  • 4. Provide education and support for patients and families regarding illness, abuse, neglect and trauma.
  • 5. Perform bio-psycho-social assessment and treatment planning.
  • 6. Identify barriers to effecting a safe and timely discharge plan.
  • 7. Collaboratively manage referrals for nursing home, rehabilitation facility, DME, etc.
  • 8. Follow up on complex discharged patients to insure excellent quality and continuity of care.

Qualifications

EDUCATION


Bachelor of Science Degree in Social Work or psychology; Masters Degree in Counseling or Social Work preferred. Two years experience in a health care setting.


SPECIAL SKILLS


Theoretical knowledge of case management and continuity of care. Hospital discharge planning skill, interdisciplinary collaboration, individual and family counseling related to hospitalization and discharge needs, crisis intervention and psycho-social assessment.


PHYSICAL DEMAND


Patient units, clinics, conference settings, offices and adjacent buildings. On call for weekends required and rotated among social service case managers. This may evolve to a part time weekend schedule if patient work load requirements mandate on-site coverage.
Alternating between office and patient units of hospital daily, as well as to adjacent buildings. Must be able to read charts and use appropriate electronic equipment.
