    Case Manager - Primary accountability for the development, coordination and management of the patient's psychiatric disposition.

    Responsibilities

    • 1. Monitor and complete discharge planning for acute care patients with psychiatric follow-up needs. Work with physician in assessing patients with psychiatric needs in the ED and work to transfer them to appropriate facilities.
    • 2. Provide support and intervention as indicated to all mother baby units identified as having a need for social service intervention including, but not limited to; paternity cases, adoption cases and protective services. Make routine rounds on unit when possible.
    • 3. Position requires weekend scheduled On Call hours as needed, which includes authorizations of addiction recovery center patients needing inpatient detoxification. Must be able to report to Hospital within 45 minutes when called in during designated On Call Hours.
    • 4. Other duties as assigned.

    Qualifications

    EDUCATION


    Bachelor of Science Degree in Social Work or Psychology; Masters Degree in Counseling or Social Work preferred.


    EXPERIENCE


    Two years experience in health care setting. Behavioral Health experience preferred.

    SPECIAL SKILLS


    Theoretical knowledge of case management and continuity of care. Hospital discharge planning skills, interdisciplinary collaboration, individual and family counseling related to hospitalization and discharge needs, crisis intervention and psycho-social assessment.


    PHYSICAL DEMAND


    50% sedentary; sitting, standing, walking from unit to unit, carrying records, speaking before groups. Must be able to speak and hear in a manner understood by most people. Must be able to communicate effectively by telephone.
